What is a school-based health clinic?
A school‐based health clinic works to improve the health of students, increase access to health care and decrease the time lost from school. The clinic provides an optimal setting to foster learning readiness and academic achievement while helping to meet the health care needs of children.
Who is eligible:
Medical: All students, staff of the Eldon school district and their families.
Dental: Open to everyone including community
What services can be provided at the school clinic?
Routine physical exams, including sports physicals
Diagnosis and treatment of illness and minor injuries
Adolescent health
Vision, dental, and blood pressure screenings
Routine lab tests
Immunizations
Health and nutrition education, counseling, and wellness promotion
Medical and mental health management of chronic conditions such as asthma, ADHD, etc.
Prescription medication when necessary for treatment of illnesses or conditions
Oral health services ‐ screening, dental hygiene instruction, cleanings, fluoride treatments, sealants, and referral for dental services
Behavioral health services including screening, assessment, conflict resolution/mediation, medication management, family counseling
What’s it cost?
COMC bills private insurance and Medicaid for eligible students. Any portion remaining is the responsibility of the family, but there are many pricing/payment options and assistance is available. You can always call the clinic or COMC’s main office @ 1-877-406-2662 and follow the prompts to the billing department if you have specific questions about payment.
Will signing up mean that we can’t use our family doctor? If we have a family doctor, do we need these services?
The clinic is not meant to replace your child’s provider. However, if you have a provider, you may still use the school services. You may find it convenient for your child to receive care if they get sick or injured at school, participate in screenings, or to receive other services. It can provide additional services to help children avoid missing school and to help you, as a parent/guardian, manage your child’s care.
Can I choose whether my child will receive services, and if so, can I select which services my child may use?
YES. The clinic will always contact you prior to any treatment your child receives to obtain your permission. Parents are always encouraged to contact the staff with questions or concerns and to be present during their child’s healthcare visit.
Will my child’s medical information be kept confidential?
YES. Your child’s health information will be treated with strict confidentiality. By signing the consent form you may give the health clinic staff and school nurses permission to communicate and share medical information regarding your child’s medical condition on a needed bases with the understanding that this information will continue to be treated in a confidential manner. Otherwise, it is your responsibility to notify the school of any medication, allergies or medical problems that may affect your child during school.
